| Dear valued Healthcare Provider, Thank you for choosing creosTMxenoprotect for an optimal treatment of your patients. According to requirements of 13A of Schedule 1 of the Therapeutic Goods (Medical Devices) Regulations 2002, healthcare providers must provide their patients with an Implant Card. Linked below, you can easily download, print and fill in Implant Cards for creosTMxenoprotect. Please note, for each size of creosTMxenoprotect there’s a separate Implant Card available. Therefore, please compare the REF number of the creosTMxenoprotect membrane you are using with the PDF documents available below and select the appropriate document to download.
